Does Mosaic Theory MD take health insurance?

No.
But we can provide a Superbill for your initial comprehensive intake.
Our philosophy centers around progressive and proactive illness prevention. The insurance based system is incentivized on volume-based sick care, which does not align with our philosophy of value-based preventative care focused on keeping you healthy rather than depending on recurring sick visits.
Moreover, we are passionate about being thorough when it comes to individualized/personalized medical care, and that requires expertise in diagnosing, assessing risk, analyzing individual data and spending time with the patient, educating them and formulating plans.
While there are “codes” to bill insurance companies when sickness has already occurred, there are no “codes” to bill insurance companies for preventing the sickness to begin with. Insurance companies do not compensate us for that expertise or time. This is why the current healthcare system is a conveyor belt of medicine - seeing patient after patient with only few minutes to spare on each, making up in volume, what is lacking in insurance payments.
The caliber of care we provide at Mosaic Theory MD can not be found in the traditional insurance payer system.

Can I use my HSA/FSA Card?

Yes, we accept HSA and FSA cards. Please verify with your specific insurance plan as to their guidelines.

Can you serve as my Primary Care?

Yes.
We believe that “Medicine 3.0” version of primary care endeavors to make health optimization/prevention its focus while cultivating on-going relationship-based care, rooted in mutual trust. Quarterly consistent continuity of care with the same physician is the best way to keep a check, so you can build peak health and stay optimized.
With on-going quarterly followup visits included in your monthly membership, your physician will help optimize your longevity journey while also providing you medication management, referrals, etc. You also have one additional 30-minute consult/visit per year that you can utilize for situations that come up (such as sick visit for UTI, Sinus infection, Cold etc).
Any additional visits (in-person/tele-visit/call) outside of those listed above, can be requested but Physician rates will apply.
We can provide a superbill to you upon request so you can submit it to your insurance company. You can also use an HSA/FSA account as well.

Will you be able to provide a superbill that I can submit to my insurance company for reimbursement?

Yes. We can provide a superbill for your initial comprehensive exam at the start of your membership. This does NOT apply to the quarterly visits that are already included in your membership.
Steps thereafter will be between you and your insurance company. We can not guarantee the amount (if any) that your insurance company will be willing to reimburse you.

What are the terms of the Membership?

Membership initiates with a one-time charge of $3499
(covers the large in-depth intake/exam/testing/discussion performed at the start of the membership over the course of 4-6 weeks; see the list of all the included components in the two grey boxes

It continues at the rate of $395/month thereafter which includes the benefits listed in the box titled “Health Optimization Continues With”,
here.

Membership can be cancelled at any time with a 30-day notice. Payments already made are non-refundable.

For the initiation fee, you have the option to utilize financing.
We partner with Care Credit to offer you the financing option.

What if I want to see my Physician in addition to the quarterly visits that are included in the membership?

We understand things come up! This is why your membership includes one additional 30-minute visit per year, on top of the quarterly visits.
You can certainly request more visits if you need them for an additional charge.
The charge will be based on the time you request:
60 mins- $650
30 mins-$350
15 mins-$175
A superbill to submit to insurance can be provided upon request.
Steps thereafter will be between you and your insurance company. We can not guarantee the amount (if any) that your insurance company will be willing to reimburse you.

What is your refund policy?

Once the assessment/testing portion of the service has begun, all payments are final and non-refundable.
If payment is made and for whatever reason the service is not rendered, 25% of the membership initiation payment will be held for the administrative work and processing fees involved, while the rest will be refunded.

All monthly membership payments that have already been made are non-refundable. You can cancel the membership with a 30-day notice at anytime.

All payments already made for the extra visits (outside of the visits that are included in the membership) are non-refundable.
