Does Mosaic Theory MD take health insurance?

No. (But we will provide Superbills - see full FAQ)
Our philosophy centers around progressive and proactive illness prevention. The insurance based system is incentivized on volume-based sick care, which does not align with our philosophy of value-based preventative care focused on keeping you healthy rather than depending on recurring sick visits.
Moreover, we are passionate about being thorough when it comes to individualized/personalized medical care, and that requires expertise in diagnosing, assessing risk, analyzing individual data and spending time with the patient, educating them and formulating plans.
While there are “codes” to bill insurance companies when sickness has already occurred, there are no “codes” to bill insurance companies for preventing the sickness to begin with. Insurance companies do not compensate us for that expertise or time. This is why the current healthcare system is a conveyor belt of medicine - seeing patient after patient with only few minutes to spare on each, making up in volume, what is lacking in insurance payments.
The caliber of care we provide at Mosaic Theory MD can not be found in the traditional insurance payer system.

Can I use my HSA/FSA Card?

Yes, we accept HSA and FSA cards. Please verify with your specific insurance plan as to their guidelines.

Will you be able to provide a superbill that I can submit to my insurance company for reimbursement?

Yes.
We provide a superbill for your Longevity Checkup Assessment.
If you enroll in our Healthspan Club membership for ongoing healthcare, superbills will be provided for any additional visits
outside of the two check-in visits (6 month & 9 month check-ins) that are included in the membership.
Steps thereafter will be between you and your insurance company. We can not guarantee the amount (if any) that your insurance company will be willing to reimburse you.


How does the Longevity Checkup work?

We provide you the choice of selecting the amount of knowledge you wish to know about your health. For that reason, we offer different levels of checkups.
After a complimentary discovery call, you’ll select the tier/level of the Longevity Checkup you wish to get. Once payment is made, our staff will coordinate the day of your visit. Your stay with us will be approximately 3-6 hours, depending on the level you choose. You will be provided your own private suite for the time you spend with us. Your physician will conduct a thorough medical intake, physical examination, and with the help of your care team, facilitate a battery of tests and assessments appropriate for the level you’ve selected. All tests/assessments are performed in-house in one easy visit.***

At the end of your visit, we’ll help you get scheduled for your “Discussion/Plan Consult”, which will be approximately 3 weeks from the day of your checkup visit with us. During this consult, your physician will go through the collected data, in-depth actionable longevity insights, and a personalized plan to get you started on your longevity journey. This will conclude your Longevity Checkup.
At this time, you can decide if you’d like ongoing support in improving your healthspan and building peak health, through purchasing a
Healthspan Club Membership with Mosaic Theory MD. Through this membership, you’ll be able to continue care with the same physician.
Even if you decide not to start a membership, you will always be welcome to visit us annually for Longevity reassessments to track your progress and optimize healthspan.

(***The only exceptions to the in-house testing are the imaging assessments that are only offered within the Ultimate package, and are performed at our partner radiology center)

If I become a member of the Healthspan Club at Mosaic Theory MD, will you also become my primary care doctor?

Yes, you can choose to utilize us as your primary care as well.
If you become a Healthspan Club member after completing the Longevity Checkup, the 6-month and 9-month check-in visits will be focused on working with you on optimizing healthspan and potentially reversing chronic-age related conditions so you can live a long, healthy, independent and strong life. Free of dependence on the broken healthcare system.
This will include addressing high blood pressure, high cholesterol, nutritional deficiencies, spectrum of metabolic conditions such as weight management, insulin resistance, pre-diabetes, diabetes, fatty-liver disease, hormone replacement etc and much more.

Membership will also allow you to request as needed visits/consults with your physician for routine/urgent primary care such as sinus infections, UTI, bronchitis, pneumonia, common cold, seasonal allergies etc.

There will be a charge for these visits/consults, based on the Physician’s hourly rate. A Superbill will be provided to you upon request, so you can submit it to your insurance company for potential reimbursement. Steps thereafter will be between you and your insurance company. We can not guarantee the amount (if any) that your insurance company will be willing to reimburse you.
(**this only applies to visits in addition to the two check-in visits at 6 and 9 months, that are already included in your membership at no extra charge)

Is a monthly membership required?

No.
However, it’s a great supplement to your in-depth Longevity Checkup.
Our comprehensive assessment via one of the three options of Longevity Checkups provides an unparalleled experience of having your health assessed. And although the Discussion Consult portion of the Longevity Checkup provides actionable insights to get you started on your healthy longevity journey, many will find that having ongoing support periodically throughout the year helps them stay on track.

Building and optimizing health is a life-long endeavor that requires consistency and daily effort.
You wouldn’t just spend one day a year on study sessions if you were endeavoring to learn a new language. You would ideally work on that skill almost daily. Same effort is required if you wish to build health and longevity that pays dividends to your later decades…and although something is better than nothing.. prioritizing health just once a year won’t cut it.

After I get a Longevity Checkup, can I get physician consultations for questions that may come up, as needed?

To obtain ongoing consults, messaging, and support through Mosaic Theory MD and its physicians, a Healthspan Club membership is required.

I do not live locally but I am willing to travel.
Can I still get the Longevity Checkup ?

Yes.
We have curated the comprehensive experience of getting an in-depth check on your health in such a way that you’ll be able to get the Longevity Checkup done in one simple visit, in one day.
You’ll spend about 5-6 hours in our facility and go through all the assessments. You’ll be provided a private suite for yourself, while at the facility. About 3 weeks after the day of testing, you can either choose to have your followup discussion consult online or in-person. The online option provides you the convineince of not having to travel back for the discussion portion.

And if you choose us for continued support throughout the year, utilize our Healthspan Club Membership option. Or, you can take the information to the primary care physician of your choosing to help you continue your journey.

**Note: The only diagnostics not performed in-house are the radiology images. This only applies to the Ultimate Longevity Checkup Tier.
Meaning if you choose the Ultimate Longevity Checkup, the CTA Angiography+ Cleerly A.I., Calcium CT Score and DEXA will be performed at our local partner radiology center. If you are traveling a long distance to us, we will work with our partners to ensure that the imaging is scheduled around the day of your visit to help consolidate your trip.

After I get a Longevity Checkup with Mosaic Theory MD, am I obligated to get annual reassessments?

There’s no obligation to reassess annually or start a membership after your initial Longevity Checkup. It’s completely up to you.
However in order to assess progress and optimize your health plan in this life-long endeavor, we do recommend ongoing support and annual reassessments.

What are the terms if I enroll in a monthly Healthspan Club Membership?

The option to enroll in the Healthspan Club Membership is available within 90-days after your initial Longevity Checkup. A lot can change in a person’s health after 3-months time. Therefore it would be inaccurate to base an ongoing plan on biomarkers that may have drastically changed since the initial evaluation, if more than 90 days have passed.
Payment starts the day of initiation.
The membership lasts 12 months.
Membership can be cancelled at any time with a 30-day notice. Payments already made are non-refundable.
An annual Longevity Checkup reassessment is required to renew your status as a Healthspan Club member.

What is the cost?

What is your refund policy for the Longevity Checkup Service?

Once the assessment/testing portion of the service has been conducted, all payments are final and non-refundable.
If payment is made and for whatever reason the service is not rendered, 25% of the payment will be held for the administrative work and processing fees involved, while the rest will be refunded.
